Model Release Terms & Agreement

ENGLISH

I hereby grant to the Photographer and Videographer the absolute and irrevocable right and unrestricted permission in respect of pictures or video that they have taken of my child or myself in which we may appear alone or with others, to copyright the same, and to use, re-use, publish, and re-publish the same as a part of their professional portfolios, or otherwise to market their services.

I specifically grant permission to use, re-use, publish, re-publish any and all photographs and video from this shoot or derivations of the same without restriction to Landscape Structures Inc. This shall specifically include, though not by way of limitation, the right to use photographs and video of myself and my child/children and/or our likeness in advertising and other publicity materials prepared for use by Landscape Structures Inc. I relinquish all direct and indirect claims or rights to photographs, printed materials, videos, or materials in any other media, which may be produced.

I understand my contact information may be distributed to the local playground consultant organization to manage who has signed a model release form or not the day of the photo shoot.  

I hereby release and discharge Photographer, Videographer and Landscape Structures Inc. from any and all claims and demands arising out of or in connection with the use of the photographs and video, including without limitation any and all claims for libel or invasion of privacy.

This authorization and release shall also inure to the benefit of the heirs, successors in interest, legal representatives, licensees, and assigns of Photographer, Videographer and Landscape Structures Inc.

I have read this authorization, release, and agreement prior to its execution and am familiar with its contents. I am of full age and am competent to sign this release. Further, I represent that I am the parent or legal guardian of the child/children, and have the right to contract in my own name, and on their behalf.

Por la presente, otorgo al Fotógrafo y Camarógrafo derecho absoluto e irrevocable y permiso sin restricciones con respecto a las imágenes o vídeos que han sido tomados de mi(s) hijo(s)/hija(s) o de mi persona, donde podamos aparecer solos o acompañados, los derechos de autor para las mismas, el derecho de utilizar, reutilizar, publicar y volver a publicar las mismas como parte de su cartera profesional, o en su defecto, con el fin de comercializar sus servicios.

Específicamente autorizo a Landscape Structures Inc. a utilizar, reutilizar, publicar y volver a publicar cualquiera y todas las fotografías y vídeos de este rodaje o derivaciones del mismo sin ninguna restricción. Esto incluirá específicamente, aunque no a modo de limitación, el derecho a utilizar las fotografías y los vídeos de mi persona y de mi (s) hijo(s) o nuestra imagen en publicidad y otros materiales publicitarios elaborados para ser utilizados por Landscape Structures Inc. Renuncio a todos los reclamos o derechos directos e indirectos de fotografías, materiales impresos, vídeos o materiales en cualquier otro formato, que puedan ser producidos.

Entiendo que mi información de contacto puede ser distribuida a la organización consultora del área de juegos local para administrar quién firmó un formulario de autorización modelo o no el día de la sesión fotográfica.

Por la presente libero y eximo al Fotógrafo, Camarógrafo y Landscape Structures Inc., de cualquier reclamación y demanda que pueda surgir de o en relación al uso de las fotografías y vídeos, incluyendo, sin limitantes, cualquiera y todas las demandas por difamación o invasión de la privacidad.

Esta autorización y liberación aplicará también a los herederos, sucesores en interés, representantes legales, licenciatarios, concesionarios del Fotógrafo, Camarógrafo y Landscape Structures Inc.

He leído esta autorización, liberación y acuerdo antes de su ejecución y estoy familiarizado con su contenido. Soy mayor de edad y soy competente para firmar esta autorización. Además, declaro que soy el padre o tutor legal del niño / niños, y que tengo el derecho de firmar en mi propio nombre y en su(s) nombre(s).
